Abstract

BACKGROUND // Multimorbidity, defined as the presence of two or more chronic diseases in the same individual, is a phenomenon that is increasing epidemiologically, according to a study in fifty-four countries that showed a global prevalence of 37.2% in people with an average age of 56.94 years, meaning, a challenging challenge for global health systems. The aim of this paper was to determine, according to the available scientific evidence, the best clinical intervention strategies to optimize the health status of patients with multimorbidity in primary health care.

METHODS // A systematic review was carried out in the bibliographic bases Medline, LILACS, CINAHL and SciELO, using the search strategy multimorbidity AND strategies OR “eHealth Strategies” OR “Primary Health Care” AND “Chronic Disease” OR “Chronic Disease Indicators” OR “health status indicators” OR “health status” OR “treatment adherence and compliance” OR polypharmacy OR “life expectancy” OR mortality OR morbidity in the period from 2018 to 2022.

RESULTS // Of the 837 articles identified, six were included for the analysis. The main intervention strategies identified were of two types: health advice via telephone (coaching type) and case management through a multidisciplinary team.

CONCLUSIONS // It is not possible to quantitatively determine which strategy of those presented is better, but it can be noted that the best strategies are those that have a multidisciplinary team, in which a clinical professional serves as case manager.
