Resumen

FUNDAMENTOS // El tabaquismo constituye un importante problema de salud pública a nivel mundial. El objetivo de este estudio fue realizar una revisión de revisiones sistemáticas sobre el coste de la enfermedad atribuible al tabaquismo, proporcionar un mapa de la evidencia disponible, identificar fuentes de heterogeneidad metodológica, evaluar la calidad de los estudios y formular recomendaciones para futuras investigaciones.

MÉTODOS // Se realizó una revisión de revisiones sistemáticas sobre estudios de coste de la enfermedad atribuible al tabaquismo. Se efectuaron búsquedas en PubMed/MEDLINE, EMBASE y Google Académico (desde enero de 1990 hasta junio de 2025). Los resultados se sintetizaron narrativamente. Se calculó el grado de solapamiento entre los estudios primarios y la calidad metodológica de las revisiones sistemáticas se evaluó mediante AMSTAR 2 y CiCERO.

RESULTADOS // Se incluyeron seis revisiones sistemáticas (publicadas entre 1999 y 2020) que abarcaron 143 estudios primarios, con solapamiento leve entre ellos. El coste de la enfermedad atribuible al tabaco se estimó principalmente en población expuesta de forma activa, a nivel nacional y con carácter anual, mediante enfoques basados en la prevalencia y considerando múltiples enfermedades, con énfasis en costes directos sanitarios. Se identificaron costes sociales de hasta 1,44 billones de dólares estadounidenses a nivel mundial (2012). Todas las revisiones sistemáticas obtuvieron una valoración críticamente baja según AMSTAR 2.

CONCLUSIONES // Aunque la mayoría de las revisiones identifica un elevado impacto económico de la enfermedad atribuible al tabaco, la comparabilidad entre los métodos de estimación es baja y la calidad metodológica de las revisiones sistemáticas sigue siendo mejorable.
