BACKGROUND // Several population-based sources provide differing estimates of childhood and adolescent obesity in Spain. These discrepancies may arise from variations in target populations, measurement methods and classification criteria, among other factors. This study systematically compared these sources to assess their comparability and complementarity for public health surveillance and policy evaluation.
METHODS // A comparative analysis of six sources was carried out: Nutrition, Physical Activity, Child Development and Obesity (ALADINO, 2023); Primary Care Clinical Database (BDCAP, 2024); National Sero-Epidemiology Study (ENE-COVID, 2020); Spanish Health Survey (ESdE, 2023-2024); Health Behaviour in School-aged Children (HBSC, 2022); and Physical Activity, Sedentarism and Obesity in Spanish youth (PASOS, 2022-2023). Data extraction was based on official documentation and structured into a matrix (general characteristics, contents and scales), defined according to the STROBE tool (2010) and an applied framework for population health surveys in Spain. Information was validated by those responsible for each source. Content comparison was recorded qualitatively (presence/absence and descriptive characteristics).
RESULTS // The sources covered different age ranges (six-nine years, ALADINO; zero-two, four, six, twelve and fourteen years, BDCAP; two-seventeen years, ENE-COVID/ESdE; eleven-eighteen years, HBSC; eight-sixteen years, PASOS); used diverse sampling frames (educational setting, ALADINO/HBSC/PASOS; individual health card, BDCAP; household, ESdE/ENE-COVID); and presented heterogeneous sample sizes (from highest to lowest and per year of age: BDCAP=35,961; HBSC=4,204; ALADINO=3,170; ENE-COVID=659; PASOS=356; and ESdE=231). Anthropometric assessment also differed (direct measurement, ALADINO/PASOS; self-reported, ENE-COVID/ESdE/HBSC; clinical records, BDCAP), as did obesity classification criteria (World Health Organization, ALADINO/BDCAP/PASOS/ENE-COVID; International Obesity Task Force, ESdE/HBSC; with comparative analyses using these and the Orbegozo references in some sources). Content coverage (56 items) was highest in ESdE (35; 62.5%) and HBSC (22; 39.3%). Regarding scales, PASOS (6 dimensions) showed the broadest scope. The findings described a modular system in which no single source covered all dimensions; rather, the existing sources addressed different research and policy questions according to their specific design.
CONCLUSIONS // Comparability across sources is limited when attempting to contrast prevalence estimates, whereas complementarity is high when the objective is to build an integrated surveillance and policy evaluation system combining precision, temporal continuity, breadth of determinants and data reuse. Specifically, ALADINO and PASOS enhance validity through standardised measurement and obesity-specific content; BDCAP provides annual clinical series; ESdE and HBSC strengthen data reuse and breadth of determinants; and ENE-COVID offers provincial-level analysis in a pandemic context.
