Abstract

BACKGROUND // Differences in the organisation of health services in the autonomous communities (AC) could be reflected in cross-level clinical coordination and point at areas and strategies of improvement. The aim of this paper was to analyse the differences in clinical coordination between levels of care and related factors in Basque Country, Catalonia and Balearic Islands.

METHODS // A cross-sectional study based on an online survey of Primary Care and Secondary Care doctors in Basque Country (n=572), Catalonia (n=2,277) and Balearic Islands (n=441) was carried out, by self-administration of the COORDENA-TICs® questionnaire. Variables of analysis were: experience and perception; factors. As stratification, autonomous community and level of care were used. Adjustment was made using sex, care level, type of hospital and of area management. Poisson regression models were used to detect differences between ACs.

RESULTS // There were differences in the experience of cross-level clinical coordination between ACs, according to type, and in factors; results were better in the Basque Country. Experience of cross-level coordination of information was relatively high in the Basque Country and Catalonia. In the coordination of clinical management, agreement on treatments and limited coordination of tests, joint plans, follow-up and accessibility between levels stood out, with differences between ACs. There were differences in organisational and interactional factors. Results were worse in Primary Care.

CONCLUSIONS // TThe differences in clinical coordination and related factors between ACs and levels of care point to the need for an in-depth analysis of the causes to identify strategies relevant to each context and to reduce any potential impact on access to quality care in the National Health System.
