Resumen

FUNDAMENTOS // Las cohortes poblacionales permiten estudiar trayectorias de salud y bienestar en la infancia y la adolescencia, así como sus determinantes y su utilidad para orientar políticas públicas. El objetivo de este trabajo fue identificar, comparar y sintetizar cohortes de base poblacional sobre salud y bienestar infantil y adolescente en el ámbito nacional e internacional.

MÉTODOS // Se realizó una revisión sistemática descriptiva y análisis comparado de cohortes con publicación de referencia de 2010 o posterior, en español o inglés, con más de 2.000 participantes y diseño longitudinal prospectivo o retrospectivo. Se excluyeron cohortes hospitalarias, locales o centradas en patologías o condiciones específicas. La identificación se llevó a cabo mediante fuentes especializadas (Birthcohorts.net, CLOSER, CLS-UCL y EU Child Cohort Network), bases bibliográficas (PubMed, Scopus, Web of Science y Google Scholar) y búsqueda en bola de nieve. Se extrajo información sobre contexto institucional, gobernanza, diseño, contenidos, acceso a datos, producción científica e impacto en políticas públicas. No se realizó una evaluación formal de la calidad metodológica o del riesgo de sesgo, al ser la cohorte como infraestructura la unidad de análisis.

RESULTADOS // Se seleccionaron diecinueve cohortes de veintisiete países. Predominaron las cohortes europeas (doce de diecinueve), aunque también se identificaron iniciativas en Norteamérica, Asia, Oceanía y una cohorte multinacional con países de África, Asia y Centroamérica. Se observó gran heterogeneidad en tamaño muestral, estrategias de reclutamiento, intensidad del seguimiento y accesibilidad de los datos. Todas las cohortes incluyeron registros clínicos y/o administrativos, once recogieron muestras biológicas, diez incorporaron información genética y dieciséis realizaron exploraciones clínicas. Predominó la financiación pública. Varias cohortes mostraron elevada producción científica, pero la traducción documentada a políticas, programas o planificación fue más desigual y estuvo claramente documentada en un número limitado de casos. La comparación permitió además sintetizar características seleccionadas de las cohortes relevantes para la vigilancia y la evaluación de políticas.

CONCLUSIONES // Las cohortes identificadas constituyen infraestructuras estratégicas y complementarias para estudiar la salud y el bienestar infantil y adolescente. Su diversidad metodológica y de gobernanza aporta lecciones relevantes para el diseño de futuras cohortes, especialmente en relación con la combinación entre cobertura poblacional, riqueza de medición, vinculación con registros y capacidad de transferencia a la acción pública.
