Abstract

Childhood overweight and obesity present an urgent Public Health challenge across Europe, impacting health, wellbeing and quality of life among children, and foreshadowing a significant future burden on health systems and economies. Reliable and comparable surveillance data are essential to inform effective responses. In recognition of this, the World Health Organization Regional Office for Europe launched the Childhood Obesity Surveillance Initiative (COSI) in 2007 to harmonize anthropometric data collection among children aged six-nine years. Since its inception, COSI has expanded from thirteen to forty-eight participating countries and has collected data of approximately 1.6 million children for over fifteen years.

Spain has participated in COSI since 2009-2010 through the ALADINO study, providing high-quality, nationally representative data. The last round (2022-2023) included 12,684 children, showing overweight prevalence ranging from 33.6% at age six to 40.5% at age nine, and obesity from 14.1% to 18.8%, respectively. Despite achieving one of the largest decreases in overweight between round 5 and 6 of COSI, Spain remains among the countries with the highest prevalence rates in the region.

This publication highlights the value of international collaboration within COSI, which has been consolidated as a harmonized surveillance system of high scientific rigour, essential for driving forward policies aimed at preventing childhood obesity across Europe. Its findings in Spain have supported national measures, improved regional understanding of children’s weight and lifestyle habits, and positioned the country as a frontrunner in the WHO Acceleration Plan to Stop Obesity, contributing to safeguard children’s health in Spain and across the European region.
