Ahogamiento intencional como método de lesión autoinflingida y suicidio: un análisis de veintiún años en Galicia

FUNDAMENTOS // El ahogamiento y el suicidio son fenómenos complejos interconectados y representan importantes problemas de Salud Pública. El objetivo de este trabajo fue identificar y describir las características y factores asociados a los ahogamientos intencionales en Galicia (España), analizando una cohorte retrospectiva de veintún años.
MÉTODOS // Se realizó un estudio descriptivo, retrospectivo y longitudinal (2001–2021) que analizó una cohorte de pacientes de entre cero y cien años atendidos por unidades de Soporte Vital Básico y Avanzado (SVB/SVA) del FPUS 061 y por centros hospitalarios del Servizo Galego de Saúde (SERGAS). Se incluyeron los registros codificados con diagnóstico de ahogamiento o casi ahogamiento, recodificados como ahogado mortal y no mortal según recomendaciones internacionales, así como aquellos que contenían los términos suicidio, autolisis o intento autolítico en los registros asistenciales o hospitalarios.
RESULTADOS // Se documentaron 99 casos, representando el 9% de todos los ahogamientos en Galicia y el 14% de las personas ahogadas mayores de sesenta y cinco años. El 82% de las víctimas eran residentes del municipio donde ocurrió el incidente y el 60% eran mujeres. El 99% tenía diagnosticado previamente algún trastorno de salud mental. Las personas mayores de sesenta y cinco años representaron el 53% de los casos, con una tasa de mortalidad del 50%. La presencia de testigos se asoció con menor mortalidad.
CONCLUSIONES // Existe una relación entre los ahogamientos con fines autolíticos y los trastornos de salud mental. En estos eventos, la presencia de testigos es un factor protector, mientras que la edad avanzada es un factor de riesgo de mortalidad. El ahogamiento intencional es un problema de salud complejo y poco estudiado, por lo que se deberían poner en marcha estrategias de prevención y Salud Pública para disminuir estas muertes evitables.
